Why a lost filling is urgent (even if it doesn't hurt)

A lost or fallen-out filling exposes the inside of your tooth — which is much softer than the outer enamel. Without the protective seal a filling provides:

  • Bacteria gets in. The hollow cavity is the perfect environment for new decay, often growing significantly within 1-2 weeks.
  • The tooth structure weakens. A tooth with a hole in it can crack or fracture, especially under chewing pressure.
  • Sensitivity gets worse. Dentin (the layer under enamel) is full of tiny tubes that connect to the nerve. Without the filling protecting it, hot, cold, and sweet hit the nerve directly.
  • You can lose the tooth entirely. If the cavity reaches the nerve, you’ll need a root canal or extraction.

Bottom line: even if there’s no pain, get the filling replaced within 1-2 weeks. The cost difference between replacing a filling vs. doing a root canal is significant.

First 24 hours — what to do

Self-care until your appointment

  1. Find the filling if you can. Bring it to your appointment — sometimes useful for matching size/shape of replacement.
  2. Brush the tooth gently. Soft-bristle brush, focus on cleaning the hollow space carefully.
  3. Rinse with warm salt water 2-3 times per day to keep the cavity clean.
  4. Avoid sugary and hard foods on that side. Sugar feeds bacteria; hard foods can crack the weakened tooth.
  5. Use temporary dental cement from a drugstore (Dentemp, Refilit) for $10-15 if you can’t see us right away. Apply per package instructions.
  6. Avoid chewing on that side. Even with temporary cement, the tooth is structurally compromised.

What we'll do — same-day filling replacement

For a standard lost filling at Eco Dental NY:

  1. Exam + X-ray to check the underlying tooth and look for new decay or cracks (10 min).
  2. Local anesthesia. Numbing the area (5 min).
  3. Clean out the cavity. Remove any new decay or bacteria, prepare the cavity walls for bonding.
  4. Place new composite filling. We use tooth-colored composite — mercury-free, blends with your tooth color. Cured with a UV light. Polished to a natural finish.
  5. Bite check. Make sure the filling matches your bite perfectly.

Total time: 30-60 minutes. You can eat normally on the same side within an hour (once anesthesia wears off).

When a filling can't be replaced — what happens then

Sometimes a lost filling means the tooth needs more than a new filling:

The cavity has grown into the nerve

If decay has reached the pulp chamber, you’ll need a root canal followed by a crown ($1,200-1,800 for crown + $800-1,600 for root canal). Symptoms: severe pain, sensitivity to hot drinks even briefly, pain that lingers after the stimulus is removed.

The remaining tooth structure is too damaged

If less than 50% of the natural tooth remains, a simple filling won’t hold. Options: build-up + crown ($1,800-2,500 total), or extraction + replacement ($3,500-5,000 for implant + crown).

The tooth has cracked

A crack changes everything. Sometimes a crown solves it ($1,200-1,800); sometimes the crack extends below the gum line and the tooth must be extracted.

How long do composite fillings last?

Modern composite fillings typically last 7-15 years with good oral hygiene. Some last much longer; some need replacement sooner depending on size, location, and how you grind your teeth.
